This form is ONLY for a student that needs to have an early dismissal or their transportation changed for one day. This form must be submitted before 1:30PM. For the safety of our students, early dismissals are not permitted after 2:00pm. Any parent coming to pick up their child after 2:00pm will be required to wait at our regular 2:30pm dismissal time on Bowers Street.
